Sparc (TVT-type) sling – Complication
Surgery: SPARC (TVT-type) mesh sling for SUI
Symptoms: Pain at the urethra, pain with intercourse at urethra, urgency, frequency, difficulty emptying bladder x 6 months (and shortened vagina from hysterectomy and A/P repair without mesh)
Treatment: Vaginal removal and release of mesh tape sling and Laparoscopic Neovagina for shortened vagina
This patient is a 57 year old who presented with complaints of bladder spasms, difficulty emptying her bladder, urgency, frequency, shortened vagina, urine leakage and painful intercourse for 2 years. She claims these symptoms developed after having surgery in June 2010.
Prior to June 2010 the patient had stress urine leakage and what she believed was only a rectocele. She saw an urogynecolgist in Georgia and subsequently underwent:
Transvaginal hysterectomy (TVH), Anterior repair (AR), Posterior repair (PR) and a Sparc sling (Retropubic Sling). After the surgery she developed the above symptoms. Her exam revealed a 4 cm vagina (normal is 8-12 cm). Testing revealed difficulty emptying her bladder with elevated post urination residuals of 150cc (normal is approx 60 cc). She was distraught over all the symptoms but pertinent to this case was the urgency, frequency, difficulty emptying her bladder, pain at urethra.
She was taken to surgery and underwent a transvaginal approach to sling revision and removal of the sling beneath the urethra. Approx a 3 cm piece of sling was removed. After the surgery she immediately noted a dramatic reduction of urgency and frequency as well she could completely empty her bladder. The patient also underwent a neovagina (Davydov) procedure to lengthen her vagina. Her vagina was lengthened from 4cm to 10 cm and now has a full functioning vagina.
